Examination:  On examination,  there was a strong positive response to the cold test and a positive response to the percussion test on tooth 36#.

A clear crack was clearly seen in the distal part of the tooth and the tooth was dark

The 36# tooth had no deep periodontal pockets, no excessive mobility, and no

fluctuating buccal swelling.

Pre-operative CBCT

CBCT can clearly show that a large number of pulp stones in the pulp chamber and no periapical leison .No obvious crack was seen in each axial cbct.

The diagnosis of 36# Irreversible pulpitis

The first step is tooth preparation followed by invasive access to root canal system. A large number of pulp stones can be seen in the pulp chamber, consistent with the embodiment of cbct.

Preparation the root canal by Rotate rotary file (MB ML: 2506.  D:4004)and irrigation by 3% NACLO.

Use "Ergo" in the process, and strictly follow the "“one stroke, one wipe” method to clean up debris to control infection

BC sealer(croot sp)+GP obturated the root canal systems by single-cone technique.

This is an important step in a successful treatment, as I do not want the canal to receive pressure from any condition.

Because of this, single-cone technique is the best choice

Next, I need to continue to deal with the cracks. I need to remove the cracks until the color is white and create some space for the fiber band.

A fibrous band was used to completely cover the crack and Immediate Dentin Sealing(IDS).

Conclusions

It must be admitted that the long-term effect of cracked teeth is not as good as that of teeth treated with root canal because of caries. However, with the continuous improvement of technology and concept, the use of fiber straps, and the progress of bonding systems, we have increased confidence in the long-term results of this type of teeth.
