In this case I had to retreat two badly decayed teeth.
We put provisional crowns first, in order to preserve the aesthetics of the patient between treatments, then I scheduled two appointments for endo + post endo + adaptation of the provisional crown.
After that, the patient had the final crowns.
The main point here is to restore the tooth without sacrificing further dentine, so, as we say, we adapt the post to the root canal and not vice versa.
Since it is well known that the cement is the locus minoris resistentiae of the whole adhesive restoration, we aim to keep its layer as thin as possible. The possibility to have an oval shaped post (Hi Rem Polygon Post, Overfibers) helps adapting the shape of the post to the shape of the root canal.
The use of a self adhesive dual curing material (Overcem SA, Overfibers) helps reducing the steps of the post endodontic restoration, while showing great adhesive qualities. I used the translucent shade for cementing the post, while my colleague went for the opaque shade to lute the crowns.
The main clinical suggestion we can give to those using OverCem on zirconia is to have the crown sandblasted by the lab.
Conclusions
The HiRem Polygon post is a great solution to restore teeth with oval shaped root canals, ensuring excellent mechanical qualities and reducing the amount of cement necessary for the procedure.
