- A 28 years old female patient presented complaining of moderate sharp pain to cold in her upper right premolar area. History of sharp pain in the same area around a week ago, when she visited a dentist who did emergency access to UR4.
- Clinically, UR4 has a temporary filling, UR5 with distal proximal decay and painful response to cold. both teeth had no tenderness to percussion.
- Diagnosis: UR4 previously initiated and normal apical tissue, UR5 symptomatic irreversible pulpitis (SIP) and normal apical tissue.
- Treatment plan: NSRCT for both UR4 and UR5, then definite cuspal coverage.
- Medical conditions: Patient is having a bleeding disorder (Von Willebrand disease). Although the treatment didn’t include any surgical approach and the patient been reassured about that, but she wanted to finish the treatment in one session, as she wanted to take the bleeding factor before the session, due to a bad previous experience with a wisdom tooth extraction.
Procedure: After Buccal infiltration anaesthesia and rubber dam isolation, both UR4 and UR5 been accessed and prepared with Rising file system by Fanta up to #30/04. UR4 had two canals, while UR5 had one canal then split into two canals.
Disinfection with NaOCl 3%, EDTA 17%, with ultra sonic activation (PUI).
Hydraulic obturation with premixed BC cement.
Composite cores for both teeth, and patient been scheduled for definite cuspal coverage.
Conclusions
Rising file system by Fanta is an outstanding file system, due to many advantages like minimal invasive shaping, extreme cutting efficiency, maximum debris removal, and different NiTi alloy in each file. These properties have enabled Rising system to be the file of choice for majority of cases.
