
Primary treatment of a lower molar with Rising

Patient came with acute pain on his mandibular right molar. He can't use the tooth for functioning, and sharp pain appears immediately when he drink something cold. Cold test revealed that the pain coming from his first mandibular molar. No cavity was found, only small composite filling on distal region.

Periapical radiograph taken to see the source of the pain. Although there was no sign of deep caries, but we can see clearly the sign of fibrotic / calcification in the pulp chamber. The suspicion of crack tooth syndrome was strong in this case and endodontic treatment was scheduled to relieve the pain.

Anasthesia performed using combination of local infiltration and periodontal ligament injection. Access cavity was made and trying to be as small as practical. From the microscope, we can see clearly the crack line from distal area and extending to the pulp chamber making the diagnosis confirmed as a crack tooth syndrome.

Instrumentation done using RISING file a new file system from FANTA Dental. This file has a concept of minimally shaping and has a coronal shaping size of 1,0 mm so we can save as much as practical the PCD area in which we need to retain specially in crack teeth. Shaping done until R2 with size of 25/.04 , activation of 6% NaOCl using sonic activation and obturation using BC sealer and single cone.

Conclusions
Fanta Rising is an efficient and conservative system to shape the root canal system
